To proceed with an application, click on the following link for a
small society lottery application form (38Kb). With this application the terms and conditions or constitution of your society need to be included. You should also provide a declaration stating that the society is non-commercial, and is established for one of the following purposes
For charitable purposes
For the purpose of enabling participation in, or of supporting, sport, athletics or a cultural activity
For any other non-commercial purposes other than that of private gain
To complete your application you will need to obtain, a Disclosure Scotland form which is a ‘subject access application form’. This form allows you to apply for access to information about yourself which is held on the Police National Computer.
This form gives details of what to do regarding completion of the form, proof of identity and what happens next.

There is a payment for this service which is currently £23.00.
