Community Hub partnership

What it means to be a partner

The Great Yarmouth Borough Council Community Hub team includes Case Workers who provide wraparound support to the Borough. The Community Hub also helps local professionals come together and collaborate around support for their clients.

Professional partners of the Great Yarmouth Community Hub can access a wide range of our support for the benefit of their clients. Partners can attend our biweekly Collaboration meetings, where the cases of consenting individuals who may need more wraparound support are discussed.

Partners also receive our fortnightly newsletter, packed with information, events and opportunities, and can use our support request form to request additional support for residents from our Case Workers

Hot desking may be available for our partners at our council premises which allows you to work within our team and other hot desking partners.  This makes for a really collaborative working space to be in.

You can also benefit from free use of our shared case management system to manage your own cases without the costs of your own IT systems.

How to become a partner

A Data Sharing Agreement must be signed by a senior manager or the person responsible for data protection in your organisation, this will be sent to you along with the other relevant documents included in the landing pack.
